I just sent an email to friends who are in Anand now and they are starting the paperwork need by the US Consulate. With respect to forum DS-2029, I was asked if details are needed on all travel. I didn't quite recall, but found this email, that answered the question:
Dear Mr. HammockGuy
Rachel has left Mumbai and moved to her new onward assignment; Nancy just left post for her new assignment; Katie switched out of ACS and Wendy will be departing Mumbai in July.
You will need to complete form DS-2029, but you may only disregard the continuation sheets if (1) your sperm and (2) your wife’s egg was used in the surrogacy. If one parent is not biologically related to the child, for example: father’s sperm and anonymous egg donor, then the continuation sheets for DS-2029 must be completed.
Photographs may be taken at the consulate for a small fee. The cost is 200 rupees for three 2”x2” photos.
